Follow-up Question Tool > Add Question and Tool > Edit Question Pages

Registry Managers can modify follow-up questions that can be sent to Registrants via the Registrant Portal on both the Follow-up Question Tool > Add Question and Tool > Edit Question pages as follows:

ClosedAdd a New Follow-up Question from the Follow-up Question Tool > Add Question Page

To add a question from the Follow-up Question Tool > Add Question page, perform the following steps:

  1. In the Manage Question section, complete the following:

    1. In the Question Text box, enter the question text including punctuation.
    2. In the Key Words box, enter key words that can be used to search for the question. To enter multiple keywords, separate each word with a space.
    3. In the Question Type section, select the type of question that will be displayed.

      Note: -_If you selected the Radio Buttons with Numeric Value question type, you must enter numeric values in the Minimum Value and Max Value boxes. If you selected the Radio Buttons with Free Text question type, a free text answer is not required.

    4. Click the Save Question button.

      A new popup window displays that the question was saved successfully.

    5. Click OK.

      The Manage Answers section displays.

  2. In the Manage Answers section, complete the following:

    1. If you want to allow the Registrant to not answer the question, select the I do not wish to answer checkbox.
    2. If you want to allow the Registrant to indicate they do not know the answer to a question, select the Don't know checkbox.
    3. In the Answer Text box, enter the text for the answer.

      Note: -_Answers for "Dropdown List" questions are limited to 150 characters.

    4. In the Order text box, enter the order that the answer will display in the list of answers.
    5. To make the answer editable, select the Editable checkbox.
    6. To provide help for the answer, enter the text in the Answer Help Text box.
    7. To make the answer part of the information displaying in the Clinically Relevant Summary section of the Registrant Record, select the Clinically Relevant checkbox.
    8. In the Key Words box, enter key words that can be used to search for the question. To enter multiple keywords, separate each word with a space.
    9. In the Question Type section, select the type of question that will be displayed.

      Note: -_If you selected the Radio Buttons with Numeric Value question type, you must enter numeric values in the Minimum Value and Max Value boxes. If you selected the Radio Buttons with Free Text question type, a free text answer is not required.

  3. To finish the changes to the question, choose from the following options:

    1. To save the question information without completing it, click the Save Question button.
    2. To save all changes for the question, including the answer information listed in the Manage Answers section, click the Submit button.

      WARNING! -_Once a question has been submitted, it cannot be changed.

    The system displays a new popup window confirming that the question was saved successfully.

  4. Click OK.

ClosedAdd a Follow-up Question from Tool > Edit Question Page

To add a question from the Tool > Edit Question page, perform the following steps:

  1. Click the Add New Question link.

  2. In the Manage Question section, complete the following:

    1. In the Question Text box, enter the question text including punctuation.

    6. Click OK.

  3. In the Manage Answers section, complete the following:

    3. In the Question Type section, select the type of question that will be displayed.

  2. To update the answers in the Manage Answers section, complete the following:

To view all of the follow-up questions outside their groups, click the Go to Question List link.

  1. Click the F/U Question Tool tab in the banner at the top of the page.
